Embodiments are described that address the technical need for an extended response / rate control channel. In one aspect, the response command and rate control command are combined to form a combined command. The combined command is generated according to the constellation of the points, each point corresponding to a pair constituting a rate control command and a response command. In another aspect, the constellation of the points is designed to provide the desired probability of error for each command point. In another aspect, the common rate control command is sent with a combined or dedicated rate control command. Various other features are also provided.
